peeling exterior paint

The exterior west wall of my house is pine- sided, about 12 years old; there's a vapour barrier on the inside. Every year this wall needs to be re-painted. I've tried different brands of paint; the latest was Benjamin Moore. I've tried latex primer with latex finish coat. alkyd primer with latex finish. Last summer I scraped it down and primed with alkyd and finished with latex, it's 60% peeled yet again.
Any advice? The rest of the walls of this house tend to have paint life expectancies in the 3-6 year range depending on exposure to sun and elements. This annual task is becoming pretty tiresome.

Is the paint peeling and leaving the primer exposed or is all of it peeling leaving the wood exposed?

Chris: the paint comes off to the bare wood.
