Yet another paint question-prep?

I was reading an earlier question that was posted. Several people noted that the prep to the wall is extremely important prior to painting. What prep work should be done?

Fill all holes and sand smooth. Make all repairs. If the walls have a shine then lightly sand - a pole sand works great.
The walls must be free of grease, dirt, and grime. This varies from house to house, room to room. eg) if you are heavy smokers, use a wood-burner a lot, or have an oil furnace then wash the walls. If you cook a lot of oily foods, then wash the kitchen walls.
If the existing paint is in bad shape, prime the walls first. If the walls haven't been painted in 15 years or so, prime them. Large repairs should be primed.
