91 Toyota PU

How do I check to see which one of the universal joints is bad, or should I replace all of them?
How can I check the carrier bearing?

I'm getting a vibration and squeaking when taking off

Sounds like wasted U-joints. I'd change them: Old truck, many miles, lots of stress.

Should be good to go then. If you grab the driveshaft in park and feel lots of movement, they are wasted (based on the noise, they likely are).

Take the shaft to a driveshaft shop or let the auto parts replace them. Auto parts charge around $15 a joint labor, they are small joints and a bear even for an experienced person

The driveshaft shop will charge more but they will balance the shaft

The shaft is factory balanced--unless you altered it, or one of the tacked on weights was knocked off--you should not need to balance it! U-joints and a carrier bearing are good insurance on a high miler!
