Car hesitates when cold

I have a 2000 toyota echo with 200,000 mile. When you first crank it, it hesitate like it isn't getting any gas. But after a couple minutes it's OK.

Wouldn't hurt to clean out the intake where the throttle plate is. Remove the flexible intake duct where it meets the intake (you should see the throttle cable nearby). Prop open the throttle and use an old toothbrush and some carb cleaner to scrub away any deposits on both side of the plate and the surrounding throat of the intake. See if that helps.

That is a LOT of miles for a 6 year old car; my wife's '01 Camry only has 50k miles. Highway?
