F-250 blower motor speed problem

I have a 2000 F250, my heater--a/c blower motor will not run on the high speed when set to heat, it works only on the first three settings. However, when set to A/C and on Max the fan does blow at high speed.

I'd eliminate the blower motor as the cause since it runs at high when the A/C is running.

The only other thing I know that could cause this would be the switch itself. This assumes that the fan is controlled separately by the heater controls and the A/C controls.

Sometimes rapidly moving a switch back and forth will cause it to operate.

One other thought: maybe it's a blown fuse. Not likely, just a thought.
