Clunking sound when braking slowly

I have a 2003 Honda Civic with 120k KMs on it. Often it has been making a clunking sound when I brake. I have made a few observations:

- Only happens in warm temperatures (a few degrees Celcius over freezing)
- Only happens if I'm moving foward ( braking when in reverse is clean)
- Only happens when I brake slowly (abrupt stopping is fine)

I have browsed the forum and although there are similar threads to this, the points above differ from them.

Any insight would be appreciated!
