Cat not using litter box
I have a 4 year old cat that has always used his littler box. He is strictly an indoor cat and a solo animal.
All of the sudden in the last 2 weeks he has starting peeing on the carpet. I have noticed it 3 times now.
The little box is kept very clean and he is healthy and has had not change in diet whatsoever.
Any ideas?

I agree...unless another animal has been there...unlikely its a behavior problem. Pets may appear perfectly healthy but still have underlying issues.

I have to agree, that it sounds like more of a health issue. Most often this happens with urinary tract infections which can be deadly. You should get him into the vet asap to get checked for this.
Do keep us posted and let us know how he is doing.

The first thing I'd ask is whether he is neutered. My 5 years old cat began doing this around 3 and half, he'd do it for around 6 months and then stop entirely for a while. Every time, we hoped it would be his last and that he'd finally gone back to normal. It only got worse.
He got a UTI back in January and had to have surgery to remove the blockage, even after that he would still pee everywhere. We finally tried having him neutered 3 months ago and so far so good. He hasn't attempted to use anything but his litter box since!
