Welcome to the forums!! I drive a Cummins, so I am giving generic advice. It is probably due to a clogged fuel filter or air filter. It's only fuel and air. You get that in an imbalance, you get boogers. You said it was good at highway speeds, it is good after it warms up a low speeds and stop and start? If it is just as you start it cold, it could be your glow plug set up. We use a heater grid to heat the air, so I'm not sure about your glow plug(s). My grid will stay and/or cycle until the engine warms up.

Where is your fuel pump (lift pump) located? Is it factory? Do you have a fuel pressure gauge. Sorry for all the questions, but it may help.
