F250 7.3 diesel - fuel pick up hesitation

I have a 2000 F250 7.3 diesel
Most of the time it runs great. But occasionally (mostly at low speeds) it acts as if there is a disruption in the fuel feed. It doesn't die but it begins to jerk as if its not getting enough fuel. When I let off the accelerator and then give it gas again it clears up for awhile. At really slow speeds (idle) it sometimes will stall. Don't drive very often as it is a secondary car. I have replaced the fuel filter.

Could it be trash in the tank or do the injectors need cleaned or replaced. 250,000 miles on truck. As I stated, most of the time the truck runs great. It acts as if there is a vapor lock of some kind
