Disk brake rotor not wearing evenly

I have a 2007 Silverado 4W with front disk brakes (drum in rear). Due a failure of the passenger side wheel hub assembly, and the mess made, I have replaced the wheel hub assembly, axle, rotor, pads and caliper. An alignment was done after this work. The problem I'm having is that the rotor is wearing with a groove in it from the pad. The groove is near the middle of the wear area where the pad contacts it. There is no debris or anything else that I can see that would be causing this wear pattern. Does anyone have any ideas what I'm missing here.

A rock or some medal got in between pad and rotor. Probably gone by now but groove will stay. Should not hurt anything.
