Unhappy Shower Water-Flow Problems

The water flow in our bathroom shower, while not a trickle, is not as strong as it should be. The water pressure coming from the spout is fine, so there seems to be a problem somewhere on the way to the showerhead. We tried the obvious solution of buying a new showerhead, thinking it was clogged, but there was no improvement.

There is also an annoying problem with hot and cold water regulation when directing water to the showerhead. There is just one adjustment handle in the center and when turning it to warm, it will at first be too hot (no cold water seems to be mixing at all) and will not adjust until it is turned back to almost completely cold. It will eventually mix to a tolerable warmth after moving the handle back and forth several times.

Both of these problems happen ONLY when directing water to the showerhead, not with the tub spout, so it makes sense that the problems occur beyond that point. What do we need to replace to fix the hot/cold mixing problem, and should we replace all the plumbing to the showerhead to solve the pressure problem? Is this something we can do ourselves? We have fairly good access to the pipes that need replacing. (We don't have much experience but are certainly willing to try.) Thanks.

What brand of tub/shower faucet do you have? You probably just need to replace the cartridge in the faucet.
