pipe hammer

My kitchen sink has pipe hammer. However - it appears to be different from regular pipe hammer. It happens only when the water is running NOT after it's shut off. Doesn't seem to mind if it's hot or cold but difficult to tell b/c it's a swivel handle that's usually in the middle. Also -it happens with zero regularity - but once it happens once, it seems to do it a couple times in a row. Will not stop until water is turned off. It can also go weeks w/out doing it. But when it does - it seems to shake the entire house.
Any ideas?
