replacing four bolt toilet

The tank on my toilet started leaking from a crack yesterday. Luckily we were home and caught it before it did too much damage. I looked all over the toilet and can't figure out who made it. There is a date of 1972 on the inside of the tank. I can get hold of another toilet, but the bad one is held to the floor with four bolts instead of two. The bolts are ten and fifteen inches out from the wall.
Three questions.
How do I find out who made this toilet, to try and get a replacement tank?
Can I put a two bolt toilet in place of the old four bolt one?
If not where can I get a four bolt toilet?

You won't find a replacement tank because they don't make it anymore... Yes a 2 bolt will work... and you will need a 10" rough toilet to fit the 10" set of bolts... You will find that the 10" set is the set in the flange and the other set is just a set for mounting that will be anchored to the floor... When you pull up the toilet, you will have to break off the spare set of bolts or just unscrew them if you are able...
