Septic PVC Cap Replacement

I have a cleanout? between the septic tank & the main line that leads to the field. It was installed when I replaced my field (about 3 years ago). It is PVC & the cap is nearly flush with the ground. I discovered a big hole right in the center of the cap. Does anyone know if these are standard & where I can get a replacement? I live in SE Michigan. Thanks.

I feel sure that a standard part would have been used. Measure the diameter. Go the HD or Lowes or whatever hardware store you have and pick up one that size. The measurements don't need to be too accurate, they come in 1 inch increments.
