American Standard shower diverter

My shower diverter recently did poorly at moving the water up to the shower head. I took it out to inspect it and it came out in pieces. The thick, white nylon washer at the end had cracked into pieces.

I found a rebuild kit at Home Depot, but no instructions on how to rebuild it. It works fair now, but water still shoots out the handle when in the spout mode.

Does anyone know the sequence to reassemble on of these diverters? I'm assuming it is around 30 years old, since the house is that old, and American Standard since Standard it stamped on the diverter handle.

Need to tighten packng nut on stem.When shower is on the plastic ring is keeping it from coming back around stem.When its turned in tub position the washer keeps the water from going to shower.The plastic ring is just hanging there at that time.If packing is loose then the water will push through around stem and through handle.
