Unhappy Flange Concrete Placement

I just had a underslab cast iron sanitary line repaired. The plumber had to jackhammer out a portion of the slab to fix the leak. When I repour the concrete floor slab, I am unsure what to do around the flange. Do You pour the concrete right up to the flange making sure you keep the flange bolt slots clean or is it necessary to form aroung the flange prior to placing the concrete. Also my exisitng angle valves for sink and toilet are flared connections. Is there a fitting for adapting flared to compression so I could go back with flex hoses rather than flared copper?

Use plumber's putty in the slots for the toilet flange, make sure the toilet flange is 3/8" of an inch above the finished concrete.

I would cut the copper and use compression type valves and use the common valves where 3/8" compression type so that flex stainless can be used.
