slab on grade

do i glue the toilet flange in before we pour the concrete? or do we frame a box at that location then install it afterwards?

NO, leave a sealed stub at least a foot above your slab. After all the work is done, and you are prepared to install your fixtures, you can cut off the stub and install your flange. It will sit flush to the floor and only stick up 3/8" which is perfect for toilet installation. Installing the flange too early will cause damage, I guarantee it.
