leaky toilet

My house is an old block & beam foundation, about 50 yrs old. I have a leak out from under the base of the toilet. So far I have done the following with no solution: had a closet flange soldered on to the existing pipe (there wasn't a flange at all); new wax ring; new toilet; new double wax rings. When I replaced the flooring, I also put down a 1/4 in. plywood subfloor first. The top surface of the flange is now about 1/16 to 1/8 in. above the new vinyl. I have found conflicting statements in my research as to whether the top of the flange or the bottom of the flange should be even with the floor surface. Which is correct and would the improper height of the flange cause leaking? A flange extender was suggested by a friend and that I thoroughly clean the old wax off the flange and toilet before I put in a new one. What else can I do to finally stop this leak before I have to replace the floor for the third time? Much thanks.

The bottom of the flange must set on finish floor, this making it 1/4" above the finish floor, at this height only one wax ring is needed to make a good seal.
