Sewer Cleanout Cap is loose... piping potentially damanged

Hi all,
i'm in the process of looking to purchase a home and one of the issues that arose when doing the home inspection was that the cap for the sewer cleanout pipe was loose. it almost looked like the piping at the top was somewhat damaged too.
anyone have thoughts on what type of repairs would be needed in this scenario? i also imagine that this is a health hazard. do you agree?

Yes that is a health hazard. The possible damage that you refer to can't be answered, I don't know what it looks like, can you post a picture?
