What are all the possible causes?
The caulking looks fine.
Could it be the Moen cartridge?

Intermittently usually means a caulking issue. Most likely place is between the pan and the tile although there should be a lip on the pan to stop that. Some people, kids especially, splash a lot of water.
You can cover the floor drain using a plastic container top, half fill the pan with water and then remove the plastic top to see if it's the drain leaking. Get water from the tub so no water splashes on the tile.
The other place it can leak is at the shower head. I would doubt the cartridge would leak that much to run down the wall.

Is the drain slow so water puddles up in the shower stall?
Does water leak down below only when it puddles up in the stall?
Can you put a switch above the shower head to stop the water? Turn on the faucets and switch off the water up above to see if the pressure build up causes leakage inside the wall. (tHe switch will come in handy to control the water more precisely during your shower.)

Are you certain it is the shower leaking? I see a tub and kids toys. Could it be water splashed out during tub time?
