Shower head extension goes behind tile ???

This is a new place. I want to change my shower head but the extension is too much of a 90 . The extension goes back into behind the tile ..
is it threaded in?? This place has plastic piping !
can I just turn on the extension??

It screwed on so it should unscrew but depending on the type of fitting used and how it's secured determines how safe it is to unscrew it.

It should unscrew with a little effort but if you have to use a lot of effort - be prepared to open up the wall on the other side to make the repair. The odds are you won't have an issue.
