Basement bath layout
Can you help clarify what the builder planned in this layout? Can I use what's here and install vanity, toilet, and tub without breaking concrete? (Will obtain permit before build, but want to decide on floorplan design first.)

The pipe from floor to ceiling is probably the vent on the top and sink drain on the bottom. The large 3" or 4" pipe is for the toilet. The pipe coming up through the cutout in the floor is for a tub or shower. If it is 1 1/2" pipe you can only install a tub. If it is 2" then you can install a tub or shower as plumbing code requires a 2" drain for showers.

You should give the area a good looking over and measure the pipe sizes before doing any serious planning. It is probably just the picture but the tub drain looks small, maybe 1 1/4" and it looks like the vent from the ceiling is 1 1/2" but the pipe/drain coming up from the floor is 2". A 1 1/2" vent just meets code which allows up to 6 "fixture units" for a vent when there is a toilet. A toilet counts as 3 units, a bath vanity is 1 and a tub/shower is 2.
