Understanding Kitchen cleanout

The picture of my kitchen cleanout at the side of the wall is attached.
Looks like there is a disk in the middle of it to prevent leakage(?)
How would I use this as a cleanout. given the center of the pipe is blocked with a "disc". I am not sure if I am supposed to break the center piece or remove it somehow ?
If I break it, how would I replace it ? Not sure I can remove it at all.

That looks like a test plug in the pipe. They are used during construction so the pipes can be filled with water or air to test for leaks. Before using the cleanout you will need to knock out the plug and pull it out of the pipe.

Thank you Pilot Dane.
The water is flowing fine at the moment, so I was just preparing for the day, i would need to snake in there.
Now I know that I can knock it out without causing any kind of issues.

It may be grease as well but looks like its not "stopping the show" at the moment.

My first thought was that it looked like grease had been poured down the drain. I have never seen even a kitchen drain that looked this bad before, but I am used to a monthly preventive measure of enzyme drain cleaner.
