a few things,

1. how old is your test strips
2. when you say you changed the water " before winter "
depending what mounth that is and how much use your
spa gets you may need to change it again,
3. to put things in perspective , most spas hold apx 300 gal
of water, most bathtubs hold 50 gal of water.
to change the water quarterly is a minimum.
4. try to get into the habit of adding a non chlorine shock,
" shocktrine " is one brand that comes to mind, basically
potassium mono persulphate removes all organic &
inorganic properties from the water, dissolves, on contact

Could be a few different thing, is your tub old and made of fiberglass? There has been old fiberglass tubs that loose their gel coat in spots exposing users to fiberglass. It could be too much chlorine if you use chlorine, or could be an allergy to a chemical in it. It could also be too hot and staying in too long. Do you have ozone? The other is PH / Alkalinity.
What I would do is take water sample to local pool store, they will test water usually at no charge. Let me know what you think...
