furnace vent on roof

Our furnace vent on our roof sits on top of a fake chimney then the vent is basically a flat piece of metal (slight slope) with welded tabs at each corner that attach to the fake chimney. This allows a space of about 3" high on all 4 sides of the vent above the fake chimney.

last week the welds broke. We've looked at some of the big box home improvement stores for replacements vents but none of them match what we currently have.

Any ideas on what we should use for replacement?

a standard heat flue roof flashing for a double walled pipe--somew2here in the chicago /illinois area I had heard of a place that makes those fake chimneys,but I don`t recall the name,you could try googling the area using descriptions

What you have is a "metal chimney chase", see for example this page at Volko Supply. The tricky part here is that the vent, chase and vent termination should all be part of a listed system, and generally you are breaking the listing if you "mix and match" components from different manufactures.
