flat roof drains leaks

I do some building maintenance on a facility that has a rather large area of flat roof with five roof drains. Recently we're having issues with water infiltrating through around these drains, dripping, seeming to be seeping around the perimeter of the rectangular plywood cut-outs for the drains you can see from inside underneath view. Also am noticing heavy condensation on the steel bottom of at least one of the drains, causing drips. Seems to occur mainly with freezing/thawing of snow on roof. Drain areas on inspection on rooftop generally looks okay with no obvious cracks, etc in roofing. Some freezing of puddling occurs around drains to some extent, guess you could call it ice damming to some extent, but nothing that necessarily looks severe (but I'm no roofing expert by any means). Here's some links to pics taken from on the rooftop and from below. If you look closely at the pics from below, you can see that the wetness occurs around the perimeter of the plywood cutouts for the installed drains. Also, I was wondering why at each drain site there are two drain openings side by side like that. Why is that? anyway here's the pics. any advice/comments would be appreciated, perhaps you have an idea of what is likely happening... thanks.
