My preference would be around 45" to 48" high. That way you'll be above a work table if you should move one in. I can't think of any code restriction on height in a residential garage.

Fished in NM can't be stapled so the code says it's ok to let it hang inside the wall. If new work you would need to staple within 12" of the box and support at a minimum of every 4 and 1/2 feet.

The GFCI is required for all 120 volt receptacles in bathroms for dwellings. There are no exceptions. Reference to 2005 NEC 210.8(A)(1). The washer receptacle needs to be GFCI in this case.
