cork underlayment with floating floor

Can you tell me how cork underlayment is applied when floating an engineered floor on a concrete slab with vinyl tile?

Sometimes it is glued down, and sometimes it can be loose laid.

Is a vapor barrier necessary?Above or below cork? What thickness cork,3mm or 6mm?

A moisture barrier, be it sheet, or coating form, should always be used over concrete. Depends if you wish to gluedown the cork or float it too, as to which one to use.
