Question about subfloor over concrete

Hi,
I am looking at installing laminate flooring in a basement over concrete. I live in Minnesota and would like to put a radiant heat underlayment called QuietWarmth under the floor. Does anyone have experience with this product?
Also, I have gathered through reading posts here that it would be recommended to put 6 mil poly down first, then the underlayment, then the flooring. Am I right? Will the heat from the underlayment be too much for the plastic? Does mildew ever rear it's ugly head when you have a plastic barrier on the cement? So many questions.

Follow the manufacturers installation instructions to the letter. You don't want a problem to occur and then someone blame you for not following instructions, even though it may not pertain to the actual issue.
