overlap reducer

I have been looking for an overlap reducer that will handle a height difference of 1/4"-5/16" and be at least 2-1/2" wide, I'm going from laminate to vinyl. It's a doorway threshold of 30" and I'm not having any luck finding them...anyone know where I should look?

Got a picture of this opening?
That wide a gap tells me one of the two floors was laid wrong.
I see it all the time with laminate that was not run straight across the opening instead of up to the door stop moulding.

I'd make my own on a table saw although dealing with the 1/16" discrepancy may be challenging.
