Lyric T5 wiring

Lyric T5 wiring

Hi guys,

I was wondering if I could get some help with wiring this thermostat. I would greatly appreciate any help. This is what I have:
Gas boiler Williamson-Thermoflo GSA 300
Lyric T5 thermostaic

I ran new wiring since the old one didn't have a C wire. I did try making some connection but all I was able to get was the Lyric powering on and cycling off. This was last year so I don't remember what connections I did. I did some research online but of course no one has a control board like mine so I couldn't replicate what others did.
Currently the old thermostatic is in place. It's connected with 2 wires (red and white). The white is connected to the Green terminal and the Red is connected to the Yellow terminal. I've included some pictures. The 2 Yellow wires have a stick that says "To 24V Operating Control" and they are capped and not connected to anything. I think I tried to connect the blue C wire to one of these if I remember.

Welcome to the forums.

Labeled your diagrams.
Using the colors you now have..... connect the three wires as shown.

Thank you. I just had an opportunity to try your recommendation just few minutes ago and am happy to report that it worked. So glad that I'm able to getting this working. Thanks again.
