bolting closet flange

I will be moving my toilet and was wondering how the closet flange should be bolted to the floor.

What type of bolts should be used? Is it ok to bolt it into just plywood or does there need to be a joist there?
For a 12" rough-in, is the center of the flange to be exactly 12" from the wall, or should I allow more space behind toilet?

Bolt to the floor, code requires you use non-corrosive screws like brass, You can secure flange to plywood.

Set center of flange from rough wall 12 1/2" - 13 1/3".
