Rough-in is the unfinished drain and water supply lines. No valves or flanges. Ususally capped for test. Rough-in for a toilet is usually 10" or 12" from the FINISHED wall to the center of the toilet discharge. Measure from the center of the pipe or flange to finished wall.

No. You measure from the center of the hole to the FINISHED wall. If you set the toilet over the hole there is a real good chance that you could be off. A half inch is a big difference and could cause bigger problems. The center of the hole to the FINISHED wall is either 10" or 12", these are the 2 standard rough-in measurements. This is also the way you purchase your toilet, 10" rough or 12" rough.

I decided it would be fun to over-tighten the bolts that hold the tank to the bowl on one of my toilets. It cracked. I need to replace it. I just need to know how to measure to get a new one that fits.

Measure the distance from the bolt that comes up from the floor to the wall. This will give you your rough dimension. It may be cheaper and easier to replace the whole toilet rather than just the tank.
