The most common cause for this is that the flapper, ball or whatever water sealing device your particular toilet has is leaking. These are usually made of rubber which will deteriorate and let water pass by it which lowers the tank's water level and causes the toilet to refill itself.Look in the tank, determine what type of ball or flapper or other type item does the sealing and replace or repair it.Usually this is very easy to do.Take the old one to a hardware store where the qualified help can match it up for you with the right replacement parts.

Along this same line, be sure the seat for the flapper is smooth. Mine seems to build up mineral deposits and causes the flapper to not seal. I use a green scotch brite pad on mine to polish it smooth.
