Toilet sprays from overflow tube

A friend of mine has a toilet that sprays from the overflow tube everytime it is flushed. Also, the bowl has bubbles coming back up after the flush. It also does not empty as completely as when you pour a bucket of water directly into the bowl.

This tiolet is in a spare bathroom that had been seldom used. Now she is using it more and noticed this problem. Does this sound like a vent issue? Trying to help a friend out. Thanks.

Is the tube from the fill valve inserted into the overflow tube and secured? Is it attached to the fill valve?

The fill tube is not inserted into the overflow tube. It is held about 1/2" above the overflow tube. It is secured to the fill valve.

More info. With each flush, the bowl will empty itself 3 times. After the bowl empties the first time, water will fill the bowl, then empties again. It will do it a 3rd time before the bowl fills and the valve shuts off.

The lady say there seems to be any water coming down from the rim. The toilet is at least 9 years old.
