Second floor toilet backs up sewage

Ever since my daughter went back to college in September, if I don't go upstairs every day or so and flush the second floor toilet raw sewage backs up into the toilet. When she's home for the weekend, it doesn't back up at all, only if it's not used for a couple of days. The first floor toilet doesn't seem to back up at all, even if I'm away for a few days, only the second floor. Does this mean there is a blockage between the first and second floors? Is it messy to have this fixed? Do they take off the toilet? I didn't know if it could back up into the bathroom when they fix it? Or is there any other way to fix it?

When does this happen? While using the toilet? Does it fill up when other fixtures are being used in that bath?
