Shower Door Installation

I am getting ready to tile my shower stall. We want a frameless shower door. I am curious though how these doors anchor to the wall.
Do they:

1) Anchor through the ceramic wall tile and into the cement board?
2) Does it require a wall stud? (I hope not)
3) Should the door anchor directly to the cement board and then I tile around it? (this doesn't seem correct)

You need to plan for adequate support from the walls and have sufficient wood behind the door to support the weight of the door. The cement board will not be sufficient to carry the load of the door. Make sure it is properly framed in advance.
