How to disassemble this faucet & what is the brand name?

IIRC, I dealt with one of those faucets. The only reason was because the handle came loose. The outermost section should unscrew. Then turn the faucets as if you wanted the water to flow. They should spin all the way off. When you turn the upside down, you'll see a screw to remove the cartridge. I don't have to tell you to shut the valves first. I don't remember the brand.

Usually there is a name printed on the underside of the spout or the back of the bezel. Those handles should have set screws on the back side. Did you look?

donoli2016 is probably right, I'm also pretty sure this is a Moen 4" spread faucet.

Both the hot and cold bell housings should unscrew CCW, letting the handle lift off. Be sure to hold the handle lever while unscrewing the housing.

This diagram shows a widespread installation, but the install is the same for 4"
