Shower Tub Faucet Valve Turns & Won't Come Out

I am trying to change the cold water washer in the shower/tub faucet.In the attached photo, I have shown the removal of the tub handle cover and stem cover. What is left is the long tube stem on which the shower handle is attached at one end and the washer on the other.
PROBLEM: The shower handle turns on and off but won't come out. I do see a big nut at the shower wall but it seems to be attached to the piping inside the wall.


Do I have to remove that nut to free the shower stem so I can change the washer? There is only this one nut and I was afraid to loosen the pipes in the wall.

That looks to be American Standard. Vintage 1959. I have the same faucet but on pink tile.

Been a while since I've worked on them but I'm pretty sure you have to turn the faucet stem while turning the nut.

That last packing nut needs to come out.
They make special wrenches for this job, any home center will have them.
