Hump in vinyl sheet flooring

Hump in vinyl sheet flooring
In one of my rooms I am seeing a couple of areas with humps where the plywood subfloor may not have totally been level or even. If I feel that area I can feel the ridge of the higher and lower sheet of plywood. My question is how do I fix that to get rid of said hump? It falls very close to a transition so I'm sure I'll mess that up doing this. Everything seems to bring out a new problem.

The subfloor should have been leveled and smoothed before the vinyl was put in. They should have placed a 1/4" underlayment down before they installed the vinyl and skimmed smooth all the transitions.
How long has the floor been down?
