Too much water when you wipe down. Will wash out the pigment.

Too much water in the mix. It will flush out the pigment ans it dehydrates.

Too much latex in the mix. Mixing and bags, instead of the entire bag, can put too much latex in the mix. Same with adding more latex to an already latexed dry mix. The latex comes to the top and leaves a milky white or lighter color.

Efforesence from a newly poured or moist concrete slab.

Or your trying to grout too fast after setting the tile. Not letting the setting mortar, completely dry first.

I'll post back if I think of anything else.

Dump the entire bag into a bucket and mix it up with NO water, then mix the batch size you want. During shipping and handling things will separate and settle.
