Pier and Beam Sub-floor problem.

I used T&G sub-floor. almost immediatly it started bulging at the seems and warping. Someone suggested I should have used regular ply wood and spaced the 1/16 to 1/8 of an inch apart to allow for expansion and contraction. They also suggested putting a layer of melomine over the plywood to cover the gaps.
How are sub floors suppose to be done on a pier and beam or any type of raised floor??????
