grout problem

I recently had a porcelain tile floor installed in my hallway. After 4 weeks the grout started to crack and come out. Also one tile became loose. My tile guy came back and re-grouted the floor. Soon after the grout started cracking again. Anybody have any ideas or suggestions? By the way, the cement was 3/4 inch thick. Is there anything I can do now or does the whole floor have to come out and be redone? I'd be grateful for any advice.

This is likely do to movement. If thats the case, regrouting is not gonna help. What do you mean by the cement is 3/4 inch thick. Give us details on the structure. Is this over a framed floor? If so, what size are the joists, what is there on center spacing and what is there unsupported span from below? What are the various layers of the subfloor, plywood, osb - how many layers and how thick. How was this cement applied?
