grout softened up

i installed tile in shower stall on floor over sand mix mortar - let set for 24 hrs. - grouted tile - let set and dry for 48 hrs. - put sealer on grout - let dry for 24 hrs. - used shower and some of the grout softened up and became milky and washed away. had to reset some tiles since water got underneath them. i re-grouted and resealed. with longer drying and setting time. want to use shower again but not sure if the same problem will occur or not. is there a special sealer or procedure i should use to ensure that this doesn't happen again?

Specifically what did you use to stick the tiles down?

Exactly what kind of thinset mortar or adhesive did you use? Exactly what kind of grout did you use? Give specific names of products used.
