Loose and cracked tiles. Best fix?

Loose and cracked tiles. Best fix?

We have one crack tile in the ceiling of our shower stall in our bathroom and one just outside the door of the shower. We moved into the house about 6 months ago. I would like to fix the cracked tile before the problem gets worse. What would be the best method of removing the old tile and replacing it?

In addition to that we also have some loose tiles location outside our bathtub. You cannot really tell from the picture but it is the lower three and top two green tiles that are loose. It looks as though somebody already tried to repair them.

I would be more concerned about why they cracked in the first place, as they typically do not when installed correctly
