New wall tile over dried mortar
I'm also installing new tile in a bath over dried thinset, but it is over a solid cement back. Do I need to apply a coat of mortar to smooth over this rough surface first or can I go right to the new mortar and tile? The surface is pretty rough.

You might want to knock the high spots off with a flat chisel or scraper. Then you can smooth it over with thin set or floor leveler.
I would probably just mix thin set a little on the stiff side and go to work.
