Outside walls Moisture barrier required?

After completely gutting an old house are you required to put plastic between the insulation and the drywall when rebuilding the walls. Is it code for all states or some?

If you use a paper back on the insulation to the room that will work as a V/B for you . But we always put a 4 mil poly on top of that to help seal the wall as a V/B then the drywall.
