Looks like mildew. Is there a roof leak in that area?

Fix the leak first, then remove the mildew/mold. If it washes off and the drywall is solid you can prime and paint otherwise that section of drywall should be removed.

btw - welcome to the forums!

If you leave the existing sheetrock in place first paint it with a stain blocking oil based primer such as Zinser 123 or Kilz. That will stop the stain from penetrating through the final water based latex paint.

If the mildew isn't washed off first you run the risk of it coming thru the primer [not right away] There are mold killing primers although I've not used them.
