Was it fitted with a loop of rebar that rusted . . . . or did they leave it ajar so that something could be slid underneath the lid . . . . some form of grappling hook. How deep is it below grade ?

I've had a cracked lid (concrete cover) on my outlet port and was able to purchase a replacement from my septic supply house as it had some fairly standard dimensions.

Your Septic pumping company must encounter this problem frequently and have some ideas (that they would share ?) If you succeed in extracting the lid and can't replace it, then wrapping it with a few loops of nylon rope or twine may make it easier to deal with the next time.
